Behavioral Health Informed Consent
If you have an appointment scheduled with one of our Behavioral Health providers, please listen to the Informed Consent information below. This information is required to be provided to all patients receiving services from our department. Your options are either to listen to the information below or read the HANDOUT. Emergency Counseling Resources

I’ve coped, struggled, and suffered with fibromyalgia for over 4 decades. When I saw the option for a chronic pain management class at St. Croix Health, I debated if it could truly make a difference, but what did I have to lose? I learned good information with practical tools to use and apply to my life. After I fritzed around for months not fully following my personal plan, I finally decided to either get serious or give it up. For 2 months, I faithfully changed my daily schedule to follow my plan and challenged myself to stick with it. It really wasn't hard other than dealing with changes out of my control. My worst flares are caused by changes in the weather. After being consistent with my pain management plan, I was amazed at the difference. I have very little to no pain and was able to immediately notice poor posture from being tense and reacting to triggers within my body. Best of all, there are no additional medications – it’s a simple routine I can follow at home. I am so grateful!
